Help identifying Class-A chimney

Trying to identify this class-A chimney.

As you can see, it has very distinct joints (very "beefy" for lack of a better word). I've done visual comparisons to all the current major brands and none of them are jumping out at me.

This is the cathedral box that supports the installation.

The class-a is recessed slightly up in the box, it appears that there was meant to be a transition collar of some type to connect the stove pipe. In this case the stove pipe was wedged up in the box and covered with a decorative collar.

I'm hoping to find the mfg. and see about replacing the support box. You can see the deterioration, I'm concerned about the longevity of the box as its supporting ~25 feet of chimney.

Any thoughts are appreciated.
