I was hoping someone might be able to tell me what model number this stove is. It was sitting unused for years with no chimney cap before I bought the house. Now that it's snowing, it might be a good time to break the rust free and set something on fire.

I dunno.
The rust don't really bother me but have you cleaned the flue or at least looked up or down it?

Should have mentioned if it is a BV4000, it will have a catalyst in it, and will most likely need cleaned before using. Check over the manual for proper operations and cleaning.

You have a nice insert there, the BV4000 was ahead of its time, one of the first "hybrid" stoves out there mixing secondary air with a cat.

I would have a professional do a through inspection of the whole setup before using, hopefully it has a stainless steel liner hooked up to it, if not I would have one put in and insulated.

I hired a "professional" to do an inspection, but he was not nearly as knowledgeable as was advertised. I spent some time today doing cleaning and rust removal and it seems to be working reasonably well, although whenever I seal the door the fire drops down to hot embers. I'm not sure if that's normal, but I'll probably break it down a little more tomorrow to see if I can find any restricted air paths.

The next problem appears to be that one of the plastic fans broke off and melted to the insert... also missed in the inspection.

When you have time, I would work on the set screws that hold the damper control on the damper, get them loose, and pull the rod all the way out and clean it up, and.. from the way it looked, get the damper out and get it welded. It looked to me like the only thing "connecting" the damper to the bar that slides it is the shoulder of the original weld.. may work forever like that, may slip out/over at a very bad time..

Other than that.. looks good, hope you are able to get it up and running again. And I also like the sliding off set box style connection to the chimney.

Thanks to everyone who has responded.

I was going to order a complete new LB101495 Fan Assembly Kit today, but A1Stoves didn't have a picture and Lennox hasn't responded to my email requesting a picture. I'm not too keen on spending $200 on something without looking at it first. I could just pay the $60 ransom for the fan blade (that probably cost $0.75 to manufacture), but that still leaves me with one plastic fan and the risk of paying another $60 later on. I'd rather replace the whole assembly and hopefully never have to worry about it again.

I thought about trying to fix that weld joint on the damper, but I have no idea how to get it out of there without pulling the entire unit out from the wall. Is it possible to tip & pull the damper down inside the fireplace once the set screws and rod are removed?

As far as the cat goes, I would like to say it looks good but I don't really have the background to make that call. I pulled it out and blew it off with an air compressor, although there wasn't really must dust in there to blow out. I'd like to say it looked pretty clean. I've had a fire going all day with the damper closed, so I guess it's working to some degree, however whenever I open the door smoke pours into the room. As a workaround I've been opening the damper, cracking to door to let the air flow in, and then adding the fuel once the fire starts back up. I don't know if that's the book answer, but it seems to work.
 
On page 14 of the manual, it says to open the bypass door and open the primary air to high before opening the door to reduce smoke into the house.

Hard to say if the cat is working without a temp gauge behind it. The manual does talk about that in page 18-19.

I guess I should have finished reading the manual. I did read the part about the temperature gauge, but what's shown in the manual and what's installed in mine are different. All I have is that little coil thing sticking out of the top (partially shown in IMG_4347 at the top of the page). It looks like a cigarette lighter on a stick.
 
Are you seeing smoke when you are burning? That is one sign that your cat is working.
There was a little smoke leaving the chimney yesterday, but I'm not experienced enough to interpret the appearance. I'll do a visual comparison this weekend (in daylight hours) before and after closing the bypass.

Yes; it seems to work better that way.

Did it stop the smoke from entering the room? I know I mistakenly opened mine up once and forgot to open the bypass and got some smoke, then I realized my mistake.

Well, I broke down and placed an order with A1Stoves a month ago for the LB101495 Fan Assembly Kit, and it finally showed up today with only one fan blade. Hopefully it doesn't take another month to get a replacement blade.

In case anyone else is looking for an image of the fan assembly kit, I attached one here. Maybe I'll be able to post some complete images with two fan blades later.
