is it normal to vent the accentra insert with 3" pipe? I thought 4" was preferred. My dealer said the 3" is what they use . I just want to be sure this is o.k.

How far are you venting? Are you venting straight out the wall? (Horizontal termination) or are you going out and up, or up and out? Harmans EVL guide, in there manual will tell you whether to use 3" or 4". Pipe is measured as follows.
90 degree or a T= 5'
45 degree angle= 2.5'
1 foot horizontal= 1'
1 foot vertical= .5'
A good rule of thumb is anything over 15 ft EVL to use 4". So if you have a Cleanout T to, 3 ft Vert to, 90* to, 3 ft Horizontal to, Cleanout T to, 3 ft Vert. You would have a 21' EVL and have to use 4". if that clarifies anything.

The insert was installed in 8/09 . My fault assuming it was 4". I just happened to stumble on fact that it's 3" when they called trying to sell me a chimney cleaning. Somehow it came up in the conversation and I questioned it and he told either way was fine.I just want to know if it''s safe and could the performance of my stove be affected. it seems to work o.k. , but i really have nothing to compare it to as far as heat output. Or, would it be wise to swap out the 4 " pipe/liner.

2 stories is a long way for 3". I don't have an insert, but if I did, it would definitely get 4". IMHO. Did you ask if they would install the 4" liner? (If you pay the price difference from 3" to 4") Good luck.
