The chimney looks brand new from top to bottom.

The hook up for the stove looks good but... The cleanout for the chimney doesnt look right. I just see block and void space in the cleanout. Chould I morter a flat bottom in this area?

Also.. is the chimney flue blocks looked offset from top to bottom. NOT BAD but a little off. This worries me that there is not cement in between them. Anything to be worried about?

Yeah, you could slap some mortar in that cleanout, I'd also smear some around where that thimble meets the chimney to fill in any gaps between. If you feel a little uneasy about this chimney best thing to do is reline with a stainless liner, you will get better performance from your stove.
 
I'm with Todd.

As you look up (or down) the liner, are the tiles all solid? Do any have cracks?

Looking down the chimney, its looks brand new... Its obviously been used but you can hardly tell.
 
If you don't line, gasket that cleanout door. You'll want an airtight seal there. A cheap way to do this is with silicone.
