I have never seen an operating insert before.

How loud is the blower fan in comparison to a bathroom fan, range fan, or window fan.

It varies by stove and they're usually speed adjustable. Medium is something like a quiet bathroom fan I guess, you notice it but it isn't really bad or anything. Of course the fact that it's blowing 650 degree air makes you pretty happy to hear it ;-)
 
Will also depend on the quality of the blower. I have a Monitor kerosene heater in the cottage. Bought it slightly used 8 or 10 years ago or so.
Worked perfect, pretty quiet for the first 7 - 8 years. But come to find out, for a period of time, Monitor went on the cheap, switched supplier of
blower motor assembly and was buying them from China. Well the Chinese maker was using the cheaest damn bearings they could find.

It now shows as the blower developed a pretty good whine. My neighgor tells me hers is even worse.
 
Go to a stove shop on a quiet weekday morning and try a few. There can be quite a difference between various companies and models. The good ones are pretty quiet. Even on full speed you can have a conversation next to one. The bad ones will drive you out of the room.
