common mistakes on wood insert installs?

after seeing how my chimney cap was installed, i would like to know what other potential errors a lazy installer might do on a insert install?
it is in a zero clearance fireplace.
i will post photos of everything when i get home tonight.
quite frankly i dont trust my install now and would like to verify it.
i burn 24/7 so any issues i need to get corrected before this season kicks off full time.
the insert was put in November last year.
