I did a search on these forums using "tax credit form" and found a gizzilion pages with the information you need. Other sources of info:
Can call the IRS
Can call the place you bought the stove from
Can talk to the people who prepare your taxes
If you use turbo tax there is a prompt that takes you directly to the form

Hello,

We bought this stove from Houseneeds.com

http://www.houseneeds.com/shop/stoves/classic_wood_stove_main.asp

The page says it qualifies for the tax credit. Does anyone know what form you need to use when submitting your taxes?

Thanks!
Gail

I received a Certificate of Efficiency for the American Recovery and Reinvestment Tax Act of 2009 (section 25c of the Internal Revenue Code) along with your receipt that is all you need. Ireceived it from my dealer.
