On top? From the bottom up I'd probably do studs, plywood, (metal), micore, durock, tile.
 
Decided to go with the Englander Madison stove from AMFM because of the ember only requirements. With that said, do I have to thinset the micore to the plywood or do I screw it directly? As squishy as the micore is, I would think directly/without thinset might be a more stable fit ..ya? No? ....then of course thinset between the rest of my layers (I.e. micore to durock, durock to tile)

Edit: this is for the sides of my hearth platform.
 
No micore needed at all for ember protection only. I would skip it. For a good tile base all you need is a sheet of cement board attached to the plywood. But if you do use the micore then no thinset is needed around the micore. Just use thinset to bed the tile on the Durock. Follow the recommended screw every 8" and the micore will be held totally captive.
 
Can't say, it is the trend, but not universally true. There is no harm in exceeding minimum hearth or clearance specs. If you already have the micore, go for it.
