You won't get much air flow through the exchanger, even with the fan removed. I have also tried removing the surround to get more heat during power failures, and it is not great. The bottom line is that you aren't going to get much heat off a flush mount insert without the fan running.

Now,if you are talking about moderate temperatures, like just freezing outside, and your fireplace is on an inside wall, and it has some thermal mass(like brick) , it might meet your requirements.
