I believe factory air settings are,

Primary fan gate fully open
Secondary two turns out on the bolts

I run my 200 with the small end of refractory towards the door.

I use a timer to shut off with coals left in the firebox.
Rake them over the nozzle
glow them with a torch
newspaper
kindling
newspaper
small splits
both doors closed
bypass open until internal flue temp is 200-250
Close bypass and gasifying

It takes less than 5 minutes from closing the door to hit gasification. If you have enough charcoal left it is so simple. About a ice cream pail will cover the nozzle well. That amount and about 2 or 3 minutes and you can have smoke free gasification.

If you have no charcoal, a couple of hand fulls of splitter trash will give you quick coals to ignite the Gases going through the nozzle with gasification around the 5 minute mark. You might have some very light smoke until you establish enough coals to ignite all the smoke/gases.

the refractory on your 100 should be oriented with the thin end toward the door . This is to deflect the fine ash byproduct that is left toward the front of the lower chamber and not build up in front of your fire tubes located in the back.

Sounds like you need to adjust the air. If you crack the top door a little does it gasiffy ? If so it needs more air.

when you adjust the air don't move it a little and say O it's not any better. Move it a good amount and then try it. It took me a few months to figure out mine wasn't getting enough air. Sometimes it would gas and sometimes it wouldn't. Now that the air is right the only time it don't gasiffy is if I get bridging.

When you say secondary "two turns out on the bolts". Are you reffering to the two bolts with lock nuts above the lower chamber door? If so these are currently many more than two turns out on my boiler. They looks to be out about as far as they can go.

Yes those are the bolts. They do stick out quite far, so that sounds normal. The turns out are not from the head. I would call Econoburn and ask dale what the settings should be on the 100.
