should we pack fiberglass around the insert? !!!!!!!!!

How does the liner get that damage, someone hit it with a hammer?
 
Where are you guys seeing a problem with the liner? The problem I see is the insert in a hacked up zero clearance firebox
 
The liner looks ok, although barely seated..
They hacked that thing all up! Looks like they even took the bricks out of it, you can’t do that. It needs removed.
 
The cold air source is coming from 2 places. The air cooled pipe allows air to flow freely, and I’ll bet there’s no insulation around the fireplace. It’s all too common, builders don’t think it needs insulated behind it, and most installers are too lazy, or crunched on time to wait on it or insist on it. We end up insulating a good part of them because we know it won’t happen.
 
It can't be THAT cold. The camera man is wearing shorts.

I circled in red it looked like the liner has a hole torn in it or something going on there at least.

That is a part of the hacked up fireplace not the liner
 
Looks like a liner into an adapter, into some sort of short flexible liner or elbow? Are you sure that is the liner messed up, and not part of the old firebox where they cut/hacked it up? Needs block off plate to start with, there is most of your cold air intrusion.

No it all needs torn out and start from scratch. It is stuffed into a zc box that they hacked to pieces. Nothing wrong with the liner just everything else
