I bought a wall thimble that has a hood built on the outside and the inside has a 2" nipple to hook up the outside air kit. Similar to the one you described but lees extensive. I paid $70 from dealer. www.Matchless.info this is the dealers web site

page 9 is Harmans answer to the outside air kit..I use it and it works great

I installed my englander using the up and out kit..........i went up 5 feet then out 2 feet.....works well,heating the incoming air is a plus.........they sell a bottom t cap so you can vacuum out the pipe, which ive used......only thing i had to do was get the angled down spout,cause when the wind blew out of the east hard enough it would snuff out the stove(on a low heat setting)..........bought mine from the Amish community we have up here and saved alot of cash(cheeper then online)

you will need to buy that kit plus the appropriate piping. i got away with the kit and one piece of 18". the only thing i need to do still is attach the air intake to the stove (right now it is just resting there). SHOP AROUND...i got my kit from EliteDeals.com ($189) and my pipe from HomeClick.com ($28)
