You will notice on page three that regular inspection of the external vent cap is recommended on a regular basis. Leaves, animals, insects and other things can obstruct the free flow of fresh air into the fireplace and the free flow of combustion gasses out of the fireplace, which can cause combustion problems.

So checking that out is a good thing to do.

On page 25, the procedure for connecting the inner and outer vent pipes together is detailed, with particular care given to completing the twist lock and sealing of those vent pipe joints. These are things installers can be lax about, and if a fireplace isn't venting properly, sealing of these joints should be inspected.

When direct vent gas fireplaces don;t vent, the cause quite often is either an obstructed termination vent cap or vent pipe that wast properly connected that has come loose internally, allowing burned combustion gasses to contaminate the fresh air needed to burn the gas.
