Sounds like something that would be custom cut and made. Maybe have an oak 4x4 or 6x6 cut diagonally in half, then trimmed to fit?
 
The stone is a 10 foot long single piece of granite and the flooting is red oak. Trying to find a matching transition piece.
Like bg said it will probably need to be custom made
 
You could pick up a cedar 4x4 at home depot and make a nice transition with only a tablesaw.

You could mill a transition with just a tablesaw and a chainsaw too using a very small oak tree, but curing it is fidgety and takes months.

You could probably find a place to buy oak 4x4 too, which would be a lot easier, but I am not sure where to get it.
