Properly installed it shouldn't, though there may be some smoke emitted by the painted finish as it breaks in. This could last for a few hot burns. With proper draft, there shouldn't be any smoke leakage.
If you are considering one of these stoves, check with the insurance company. I am not sure they are allowed to be sold for home use due to the lack of EPA certification.
