Question about grates and firebricks in inserts

Does an insert have to have a grate or do you burn the wood right on the firebrick? Never thought about it til today for some reason. I assumed you would use a grate but the inserts I have looked at never seem to have one. Which is it?

Also, regarding the steel stoves that don't have firebrick linings, do they need a grate?

While I'm asking, I assume the firebrick lined inserts are better than the steel boxes. Is this correct?

Mine has firebrick, no grate.

No ash pan, either.
 
Many stove makers advise against the use of a grate. Too much potential to over-fire the stove and most inserts are tight for space to begin with and so don't really have the room for a grate.

Some people will burn N/S or even alternate to get the effect of a grate.
 
Depends...check the online owner's manual. Mine says never use a grate (but it's not an insert, either).
