Replacing an old, worn out Heat form

I first thought this was a "heatilator" but with a little hearth.com research I see it is actually a "heat form"

Anyway, mine is worn out, and rotted out in my family room. I already have in insert on the other side of the wall to this open fireplace. It's just one huge brick mass, two flues, insert in one, rotted out steel fireplace in the other.

I would like to replace this heat form if it's at all possible. It would be nice to have an open fireplace again.

Has anyone done this?
