I am going to get an insert installed and need to add about 10 inches of ember protection, maybe thermal too

My hearth is raised 5 1/2 inches above the finished.

1) Is a fire resistant rug code for ember protection?
2) if I add a tile over 1/4 inch cement board on the finished floor, does this meet code?

1- no, must be permanent
2- yes unless it has an R requirement. If you have an R requirement you need to use something like microcell (I think that’s the name). Tile and cement board have very small R values.

A nailed down piece of sheet metal satisfies the ember protection

The stove manual should include the R value needed for hearth extension.
