If you are planning on getting an EPA compliant stove, all the information you need will be in that particular stove's installation manual.
Older stoves will have to meet the specifications dictated in NFPA 211.

You can start here;

Do you have a stove or chimney? They have to be sized correctly for each other.

Indiana building code is based on the ICC 2018 Family of Codes which requires all appliances to be UL Listed. Codes pertaining to stove and venting are under the ICC Mechanical Code. Listed stoves will have a tag with clearances given as well as in manual. Codes are for new installation, not existing. Existing installations must conform to NFPA 211 and any local codes adopted by the jurisdiction at the time of installation.

No one can tell you what hearth to build or clearance/ shielding required until they know what stove you’re installing. UL testing is what determines clearance.
