durrock behind the regular 24 guage stove pipe?

hi all, great site!

we are ready to install class a pipe this weekend and i have a question about cutting the hole in the wall for the thimble.
do you have to put durrock over the wall before you cut the hole or after. using 1 1/2 in. spacers behind durrock, regular 24 guage stove pipe 18 in from wall or durrock? does wall shield have to be behind the hole in the wall or just behind the upright portion of the stove pipe. i believe the durrock should be at least 8" above the stove all around but what about the stove pipe?
please help?
