Assistance Identifying this Insert, Please...

Trying to find out who made this fireplace insert. Came in the house, pretty sure it's a slammer but haven't pulled it yet. I anticipate needing to line the chimney and adapt the stove to it to make it functional but am really curious of the origin of this insert and would love to get a manual for it (should there be one out there).
