Can anyone identify this stove?

This fireplace/stove is in a cottage we just purchased. The insurance company wants information on the make/model. Does anybody here recognize it?

Any comments on how this thing will heat? Eventually I plan to replace it with a more modern design.

That is more for aesthetics than heat. It ain't gonna heat very well unless your standing near it, and most the heat will go up and out the stack. That thing calls for 36" clearance to combustibles. I hope there ain't no combustible materials behind that brick wall?
 
If this is a fairly recent UL approved unit the rear clearance is 24". In the manual they permit the rear clearance to be reduced to 12" if there is masonry over cement board.

One odd note in the manual - the firebox floor refractory is mixed and poured in on site. Needs 48 hrs to set up.
