Does anyone know where can I buy this chimney cap?

Hi, I'm trying to replace an old, leaky chimney cap for my woodstove. I have found multiple examples of this particular style online (the one on the far right in the picture), but can't seem to find the manufacturer's name or where I can buy one. Can you guys help? Thanks in advance.

Great, thank you both for your help. If anyone has used this particular cap and/or the related top plate (https://www.rockfordchimneysupply.com/terra-cotta-top-plate.php), can you let me know how it's working for you and how challenging it was to install? Thanks.
It's easy...scroll down on my install thread and you can see how the top plate goes on and secured to the liner and existing flue tile https://www.hearth.com/talk/threads/drolet-escape-1800-insert-install.182495/
 
Thanks for the replies. I am having trouble finding one of these that will work for my 16" x 16" clay flue with a 6" liner. The biggest ones seem to be 13" x 13" or 13" x 18". Does anyone have any suggestions and do I have a non-standard or oversized flue?

Just get a flat 18x18 plate and caulk it down.
