Help Identifying Fireplace

First post here. Need help identifying the fireplace in the new house. It seems like a poorly designed insert with blowers on the outsides, but the inside is like a standard fireplace. Likely from the early 80's when the house was built. Can you help identify what it is?

One door is missing because the glass cracked today.

Looks like an old Heatilator, but that's just a guess. Often there is a UL testing label on the inside of the door frame that identifies the fireplace maker. You can usually get replacement glass at a decent local glass shop. Be sure they know it's for a fireplace. If not www.onedayglass.com offers good service and sells ceramic glass.

I agree. Looks like a heatilator.
The four vents are not connected to the insert body and to be positioned by the mason according to the separated masonry channels he builds inside.
Never saw anyone use that configuration and for good reason. It's wrong. Going by memory the heat outlets should be at least eight inches over the fireplace opening and both sets of vents should be at least 4" off to the side of the insert.

Only odd thing is all the heatilators I've ever installed had four 8"x8" vents. Those seem taller.

This is a Majestic pre-fab unit from the late 70's early 80's.
