Inserts with low side clearances

I'm having a hard time finding inserts that would work in my masonry fireplace - because it has built in wood cabinets on either side.
Any ideas for an insert for ~2K house with low clearances on the side.
 
The more flush the insert the better its side clearances will be. Can you provide some more info on the fireplace dimensions and distances to the side cabinets and mantel? A picture can help too.

Edit: found this picture in an old thread. Are these the cabinets? How far is it from the cabinet to the center of the fireplace? From the looks of the setup, the odd overhang above the fireplace is going to be much more of an issue. But perhaps it can be protected with a mantel shield? Or are you still thinking of demolishing that overhang?
