Tax form for stove tax credit?

Sorry to do this - there used to be a sticky, but I'm a procrastinator with tax stuff. Anyone have a link to the form for claiming the tax credit for new stove installs? Thanks! Cheers!

Form 5695 line 3a is where your total amount spent goes. 30% of that amount (up to $1500) goes on line 52 of your 1040.
