Hi,
I just brought a home with a pellet stove, never used one before. Anyways, it vents through a cathedral ceiling and a bit of smoke exits near the top (yellow arrow) upon start up. Any idea what or if this can be patched? Or how difficult it is to replace. Im not even sure what this part is called?
Thanks.

That looks like it was left over from a woodstove, and is not a proper pellet stove vent which will be double wall 3 or 4 inch.
Most pellets stoves can be vented out the exterior wall. Take a look at the manual for your stove.

It looks like an adapter is inserted into the chimney and may be leaking .
First, I would remove the 3-inch pellet stove pipe, the round plate and the adapter.
Then you can see what kind of chimney you have . If it is a Class A insulated wood
chimney and the adapter is still good (Inserts into the chimney tightly, just put a
Little Hi-Temp silicon around the joint, put the plate on and reinstall the smoke pipe
Test the unit
If it is some other kind of chimney, take some photos so we can see and someone
will come up with a solution.

Keep in mind the stove pipe will be pressurized by the pellet stove combustion blower, and it will emit smoke from any hole/gaps in the piping. That is different than a wood stove, where the stove pipe will suck air from the inside once the draft is set up through small gaps.

Even with the correct double walled pellet vent pipe sealed with black RTV on start up we would always get a few wisps of smoke out of a few joints.

I really like this 4" Excelpellet venting they installed for me. It has built in gaskets. One thing though, it seems to be difficult to take apart. I tried taking off a 90 next to my nozzle outside and decided not to. I managed to get it clean all assembled using the two clean out tees.
If there was going to be a leak on mine, it might be the round cap plate on the bottom of the tee's. It's neat how those work with the screw and bracket.

That set-up looks OK, and as said on startup up a small puff of smoke is ok
Remember, a pellet stove controls the amount of heat going up the pipe
Only in rare cases can you not put your hand on the pipes, hot, yes, but not
enough heat to cause a fire. I have been using my stove for 23 years now,
and though I spent hours sealing every joint, I still get on startup a smell of smoke
It disappears very quickly
