What should be the correct distance between the top of the stainless steel flex pipe and the square metal chimney cap? The cap is attached to a 12x 12 tile flue .I noticed on mine there is only about two inches between the top of the pipe and the cap? Is this enough breathing space for the insert? Will it cause creosote accumulation? Thanks in advance for the help.

I would whack a few inches off of that liner and let that stove breathe.

Like BB said, the liner should be even with the top plate collar.
If you have a angle grinder use a cut off wheel to trim the liner even
with the collar.
