What determines the size of a flue?

I've read the VC Defiant non-cat can use either a 6-in or 8-in pipe. I hear that the 6-in does not fair well at high altitude locations.
I've read that add-on furnaces with 7 cu ft boxes can still use 6-in flue, but other free standing boxes with > 3.5cu ft require an 8-in flue.

What gives?
