Help to Identify old ZC fireplace

Please help me identify this old fireplace. I believe it’s a ZC unit but cannot find the manufacturer tag anywhere. I’ve searched everywhere I can think of on all sides, inside and out. The unit has an 8” round fresh air duct under the unit for external fan. Appears to have an 8” chimney.

Any help at all is greatly appreciated!
