Securing hearth to floor

How is a limestone hearth usually secured to the subfloor? Its just a one piece slab of limestone. Does the material used add any thickness?
How much does it weigh? If it's a wood subfloor there is going to be a lot of friction regardless. Some PL Premium would be my choice if required.
 
Don't have the exact weight. I'm sure it's heavy. Its 72" wide", 20" deep, and 3" thick. The subfloor is wood
I don't think it's going to move. The issue might be how it meets the rest of the fireplace, and how they get rid of any gaps. They may want to add a few spots of adhesive just to make sure it stays put.

If the carpet and pad are about 0.75 in thick, would you make the hearth the same thickness if you want a flush transition?
