So I was checking my measurements before pulling the trigger on my new insert and I found out that my hearth has a 4 degree slope over 18" inches. I will need to rebuild it to make it level to support the new insert. My question is there any reason to have the slope? It is in a Colonial style house built in the 80's with an exterior chimney.

Every insert I have seen has leveling bolts on the rear. If your hearth will have the proper clearance and r-values once the insert is in place then there should be enough adjustment to level the insert. MIne didn't and I had to construct a free floating hearth and set an level the insert on pavers inside the fireplace.
 
Shims?? As long as the basement does not have an equal degree of slope?? If your home is square and not in jeopardy of sagging with added weight then shim it.
