What do I have here??? (Country Flame insert)

Lots of smoke up the chimney since I last logged in!! (8ish years _g )

Have a new place with a Country Flame insert that seems to be in decent shape. Anyone know what I have??

It's an oldie. Maybe an FPD 400?

Not sure, 1970s would be my guess.
