Is it critical that the mantel where the insert sits is the same height as the firebox? Does the liner tube need to be sealed to prevent air movement up into the chimney?
 
I think you mean the hearth, the mantle would be above the fireplace. No it does not need to be the same height of the insert. Some inserts may be flush mount, ones that stick out might have a platform that can be purchased to hold the front up (I know some do).

If you run a full liner up to the top and seal the top with the top plate you do not need (by code) a block off plate down below. Members of this forum highly recommend it and some have stated they notice more heat output after installing one. It keeps the heat in the room instead of heating the full length of the chimney up around the liner.
