I am having a tough time nailing down the exact model of stove I have.. top plate is 24.5". 28.5" from back wall to front edge. seems like a fireplace model but not quite 5 bricks wide.

Fisher Fireplace Insert.
A Grandma Bear is 5 bricks wide, Insert is 4.
An insert uses Grandma doors with the same size door opening.

I had a Glacier Bay fireplace insert stove similar to your stove. I heated my house with for 15 winters. I installed it on a brick hearth on 2 bricks under each of the 4 corners of the stove instead of welding on legs. It heated great. The only reason I stopped using it was that I got a great deal on a hardly used Fisher Grandma stove .

Is that where it is going to be installed?

it was, and that's why i think i need to move on. the bulk puts it too close to combustibles even with the cement board heat shield.
Your hearth I'd nowhere near adequate either
