Step-top steel box stove ID — pinwheel draft caps + brass filigree

Hoping someone here can help me ID this wood stove. I've struck out on the stove itself — no UL/data tag anywhere on it, and so far no readable casting marks or embossing.

What I can tell you about it:
- Rectangular welded-steel box stove with a stepped top (a lower front "cooktop" plate and a raised rear section).
- Single front door with a glass window that has notched/scalloped corners.
- Two cast draft caps shaped like pinwheels/roses.
- A brass/bronze filigree scrollwork panel set into the riser between the two top levels, with two facing bird/serpent-head terminals, not sure if this is original.
- Single-wall pipe out the top rear. Stove is in Utah (Intermountain West), in case regional distribution is a clue.


Main questions:
1. Any idea of the base stove's maker/model?
2. Are the rose draft caps + brass filigree a known aftermarket kit, or factory to some brand?
3. With no UL tag, am I right to treat this as an unlisted stove for clearance purposes (36" to combustibles per NFPA 211)?
