Help identifying fireplace

Vista, CA
I have a home build in 1989 in California (Vista CA 92081 area). The hosue has 2 fireplaces that are from the same manufacturer, one of them the glass has shattered and I need it replaced. I cannot for the life of me find any identify mark or serial number or any plate whatsoever on this fireplace to figure out where to get replacement parts.

Can anyone help me identify the make and model? Or let me know where to look in this unit to find that info? thanks so much for your help.

There is a small paper notice on the inside of one of the fireplaces that we don't use, I have attached a photo of that here as well.

Often it is on a small tag in the door frame.
