Yes. Its been pushed off its perch and is sitting sideways on top of the fire. I just cleaned the pot yesterday and started the stove. It looked fine before I went to bed. I burn the Cumberland primarily. I just burn the Harman went its particularly cold so I dont have to run the Cumberland full tilt.

You have some odd problem! Definitely not indicative of the Accentra however and that buildup looks like a neglected stove. IMHO. Was this installed professionally, new, with all codes adhered to with the flue? Flue height? Clean stack?

Most likely operator error. I say that because I am guilty of almost the same thing. Did a cleaning, put altogether and burned for several hours before retiring for the night. Woke up next morning with center medallion laying in ash bin. I'm sure I didn't put center medallion in correct as I had difficulty for some reason assembling that time. After further inspection I noticed I didn't seat heat deflector on burn pot all the way.

That happened to me 3 days in a row before I finally gave in and shut the stove off for a good cleaning. With mine, it was ash build up on the rim on top of the burn pot. It kept it from seating.

I suggest you review the cleaning videos.
Each piece has to fit within another. The medallion slides up and in and holds everything in place.

medallion slides up? Are you saying that it slides up and the flame guide slips under it and the weight of the medallion holds the flame guide in place? Never heard of such a thing. Must be a newer model feature. This stove is a 2005. I'll check that when I get home but I'm pretty sure that's not the case with this particular year model.

I know the 2012 freestanding doesn't slide. Flame guide slips into place, but the sheer weight of it shouldn't be movable from ash pushing out over the edge. Unless there is some ungodly amount of build up in there (clinker speed bumps) forcing the pellets straight up rather than forward and up.

Check that you burning with no air leaks other than the intake. Check gaskets, hopper lid gasket etc. that they are sealing.
Must be the week. woke up to the same thing then happened again a day later after a good cleaning. I dialed my feed rate back to 3 and it has not happened again. Only theory is the cold weather was pushing the stover harder and longer than previously and I was getting a build up of pellets sufficient to push it off. I have a fs Accentra as well.

Just to make sure everyone is clear a clinker and the Harman speed bump are two different things. The Harman speed bump is carbon as far as I know.
Yeh it's definitely carbon speed bump. I haven't had a clinker in the Harman yet. The Cumberland gets them, but it doesn't get any carbon. I use the tool that came with the stove. It works pretty good but its getting a little dull so I'm going to get a long handle gasket scraper and bend it to suit.
I'll do the dollar bill test. The hopper gasket is good. I'm not really sure what other gaskets there are. I suppose behind the burn pot. I dont recall there being anything around the combustion fan.

Carbon buildup on the Harman can be deceiving. You think you got it because it is smooth but in reality is still there. I use a chisel and it cracks and then can see the layer still there. It can be easily missed. I scrape my pot at least twice a day and I think it smooths it over and settles in over time creating the build up. It looks like the bottom of the pot but isn't.
