Need Help With ID

The house we bought was built in 1996. I assume this came with it. Here are photos of the stove from various angles. I need help identifying it so I can get proof of UL listing to the insurance company. There are no metal plates anywhere that I can find, and it looks like maybe there was a sticker on the right side at some time.

I have tried several AI engines with absolutely no luck, so if anyone here can help, you’ll be my hero forever.

This looks like an insert to me, not a free standing stove.
I.e. normally (if I'm correct), this would be stuck in a masonry fireplace up until the little vertical step on top, with the rest sticking out.

Oh. Is it even allowed by code to do it that way?
Generally inserts have not been tested for use like that,. meaning that the UL listing is only for the use in which it was tested.

But first you need to confirm what it is...
 
Were you able to check the backside? My 1992 Arrow stove has a plate on the back side near the bottom with model number. It's not an insert-looking stove like that though.
 
Stove/insert aside what is it using for a chimney? What are your plans for it? Do you plan on using it to heat with? might want to consider removing it for insurance and get a modern stove if the chimney is good. If not you might want to scrap all of it.
 
This is an insert. It is meant to go in a fireplace. There are no tested clearances as installed. As is, it is definitely an insurance company and code no-no.

Its an older Quadrafire 3100 insert prob from the late 90's, early 2000's. It will definitely have a listing somewhere on it, or it should. Only to be installed in a masonry fireplace, or approved zero clearance fireplace. I saw a couple of air tubes in top, but no upper baffle, could not tell if it was installed? Early 3100I had the 3/8" steel baffle and kaowool blanket, later ACT units had ceramic baffle/kaowool blanket. One of my first stoves was a 1996 Quad 3100 free stander, I too got that Gold plated door delivered by accident, it was a crazy 400 dollar upcharge! The 3100I was a nice insert, if installed correctly... Good luck
