Printable form to use when measuring for inserts?

Printable form to use when measuring for inserts?
Is there such a beast?
You know like what do you need to know when looking for an insert?
It would be nice have a form to print and fill out.
So if no form what dimensions do I need?

I think that if you have the width (front and back,) height and depth, that should be enough. You should be able to look online at the make/model and the requirements will be listed on that page or in the manual.
