Do reflectors really work with open fireplaces using wood?
Do they really reflect that much heat out of the fireplace without
a blower?

The quick answer is no.

The problem with open fireplaces is that the need soooo much air flow to burn without the house being smokey that they wind up sucking out a large portion of the heat they provide to a home.

The best solution in most cases is a wood stove placed in the fireplace, or a fireplace insert; either with a chimney liner to suit the stove.
