Specifically, I'm wondering about the motor and grate assembly that has a rectangular metal air passage. There are also some vents on the top and bottom that can be opened and closed.

It's most likely original to the home which was built in the 1960s.

Bottom control at the grille is the fireplace air intake. The upper control may be for the flue damper. Shine a flashlight up the chimney and check damper operation with the control in the up (closed) and down (open) position.

Not sure if the blower assembly was offered as an option or if it is an aftermarket add-on. The U-shaped tube is a duct in which the air gets heated by the coals on the fireplace bottom. I don't find this blower mentioned in the original manual.
