Trying to identify brand of this wood stove...

I need to figure out the make/model of this wood stove from a picture (if at all possible), for an insurance quote.

I unfortunately only have this picture and can't look at it in person to get additional details. Anyone have a guess?

Looks like an old Preway Provider. The company's long out of business, but installation clearances, etc. can be found on the listing label on the backplate.
